The forms below are provided for the convenience of parents/carers, members of the public and students to easily submit forms electronically any day at any time. The forms will be automatically directed to the appropriate staff.

| Permission for Minor excursions and activities | This note gives permission for my child to attend minor excursions and school activities on the adjoining council oval or within walking distance of the school. |
| Permission to view PG Movies | From time to time during the school year students in Year 3-6, have the opportunity to watch movies, informative videos and historical footage. Some of this material may have a PG rating. |
| Permission to Apply school supplied sunscreen | Please complete the permission slip below if you would like your child to apply school supplied sunscreen. |
| Permission to Administer Prescribed Medication - Short Term | Completing this form will authorise staff at Yarrawarrah Public School to administer the prescribed medications short term. |
| Parent referral to the School Counsellor | Please use this form to refer your child to see the School Counsellor. |
| Student Special Religious Education Participation | To add or remove consent for participation in Special Religious Education. |
| Student Application for Extended Leave | Use this form to apply for extended leave from school for 5 days or more. |
| Student Digtial Technology Agreement | The purpose of this Agreement is to ensure both students and parents understand the requirements regarding Digital Technology Policy. |
